If one of the priority actions is 'Prepare the patient for surgery,' that is usually the WRONG answer because that is not up to us. Urine/Stool problems will kill a patient SLOWLY. This is LESS of a priority than other things. Think 'what will save your patient RIGHT NOW/TODAY'. Know your labs. Prioritize your labs.Prescriptions means orders. Questioning which prescription: you can question 'ambulating the patient'Top 5 Nursing Interventions (What is the priority/next action Questions)HESI prioritizes interventions in this order.

When you are figuring out what to do, find out where you are in this list and do the next intervention. Check your patient. Monitor, assess, ask Hx. Assess for signs of distress (low BP, etc). Change patient to a better position if their current position is not adequate. Patient is prone then becomes dyspnic. Elevate the HOB before anything. She said 'HESI will rarely ask you to position in unusual positions (trendelenburg) or above 45 degrees. Administer O2/Obtain O2 sats. She said 'HESI will never ask you to go above 6L. Anything above O2 at 6L is the wrong answer.'

I'm telling you, I've personally disagreed with hundreds of HESI questions. But whether I agreed or not, my school still made me take HESI exams every semester. And so I had to learn how to answer it as 'what does the Hesi test writer want' as I said before. I had to adopt the 'Hesi principles' just to pass the exams. I'm simply sharing those Hesi principles now for the sole purpose of passing Hesi.

This was an example question straight from a HESI prep book. My interpretation of the HESI rationale is: you already know RR rate is abnormal. You have enough evidence that there is distress (diaphoretic, pale, and mainly RR). Taking time to get vitals is NOT gonna help your patient. You need intervention!And even if the patient is having an MI, what's the intervention for an MI? Oxygen, apairin, nitro, morphine in that order.

So it's still oxygen. But agreed I'm not sure how much 2L more is gonna do. But that's the first action they want.Trust me, there are far worse questions lol. The answer is wrong though and a question like that should be thrown out.

It isnt a bad question because it is one of those 'well everything is right' or 'you would do everything at the same time' kind of question.You need a full set of vitals (besides maybe temp). If their sat is 100% on 2L and you turn up the oxygen to 4L, what are you actually changing? You cant make them sat over 100%.Starting at a full set of vitals allows you to see if you need to turn up the O2 or not.People typically dont get sweaty and pale when they need more O2, they get cyanotic. Sweaty and pale is a sign of poor perfusion (pump failing because MI)Also putting oxygen on someone who doesnt need it has been potentially linked to worse outcomes and isnt really used anymore, but that is a different story for a different time.

Critical.care is always unstable, and being unstable means one of the ABCs are compromised.I got in the bad habit of reading patient profiles and thinking 'what do they have'. I was wrongfully diagnosing and trying to think of a complex solution. But the answer is usually as simple as 'give normal saline' or 'insert a large bore IV' which is the IV for massive hydration.More specific concepts:For snake bites or poisons, you hydrate as well to dilute the toxins.If someone's electrocuted, the body muscles are damaged and it releases myoglobin, which may clog kidneys!

So worry about kidneys.If someone suffers from crush injury, intracellular K+ is released so worry about hyperkalemia/dyarthtmias.
